Senior Clinician Community Health
The Senior Clinician Community Health is a pivotal role within our organisation, responsible for delivering comprehensive support to patients with complex health needs.
Our ideal candidate will possess extensive knowledge and experience working with consumers with chronic and complex illnesses, including advance care planning discussions.
* Care planning and advance care planning
* Palliative care needs assessments and collaborative treatment care plans
* Referrals to relevant internal and external health and community support providers
* Education/support to other clinicians in supporting complex patient needs
As a Senior Clinician Community Health, you will be responsible for providing high-level clinical leadership, consultancy, education, and support to individuals, families, allied health and nursing staff, and other health professionals.
Key selection criteria include:
* Current Registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA
* Extensive knowledge and experience working with consumers with chronic and complex illnesses, including advance care planning discussions
* Experience in care planning and case management of patients in the community with chronic and complex illnesses
* Understanding of specialist palliative care, a palliative approach, and community health services
* High-level interpersonal and communication skills, including written, verbal, and computer skills
* Clinical leadership capabilities with the ability to provide consultancy, education, and support
* Current unrestricted drivers' licence and ability to drive/travel as required
