Key Role in Patient Transitions
The Department of Health seeks experienced and dedicated Clinical Nurse Consultants to join our team as Residential Aged Care Home (RACH) Liaison Nurses. These roles are pivotal in ensuring seamless transitions for patients moving between hospital and residential aged care homes, providing expert clinical advice, and fostering strong partnerships with aged care providers.
This role offers an exciting opportunity for RACH Liaison Nurses in Tasmania's hospitals, working with Hospitals South, North or North West Integrated Operations Centres (IOC) and Hospital Aged Care Liaison Teams (HALT).
Responsibilities
* Work in partnership with IOC or HALT Leadership Teams, clinical staff, and Private Providers to assist patients and their families navigate the pathway from hospital to a RACH.
* Provide expert clinical leadership and advice within an interdisciplinary team.
* Conduct comprehensive patient assessments and coordinate care plans for transfers to RACHs.
* Collaborate with RACH providers and General Practitioners to ensure continuity of care.
* Utilise and evaluate models of care and service delivery based on evidence-based practices.
