About the role
The Flexible Assertive Treatment Team (FACT) sits within the community of Orange which facilitates access to a broad range of clinical and non-clinical services to enable people to establish, re-establish or reclaim a hopeful and meaningful life in the community.
FACT team services are delivered by a multidisciplinary team who provide ongoing recovery oriented assessment and treatment through case management, aimed at improving the quality of life for older people with mental health needs living in the community. With a focus on hospital avoidant community-based care and cooperative transfer of care for people leaving hospital into community mental health care. We provide patients a more flexible approach at times of need.
