About the role

Permanent part time (3 days/week) opportunity, based in either Hobart or Perth. Reporting directly to the National Manager Connecting Peers Program you will be responsible for:

* Building collaborative relationships with program stakeholders, including people living with dementia and their carers
* Conducting participant screenings and registrations into the Dementia Australia Peer Support program
* Using peer-to-peer match policy to provide suitable 1:1 connection between Peer Leaders and Peer Participants
* Ensuring on-going monitoring of participant pathways, including each peer match progression
* Participating in and contributing to service evaluations, planning, development and innovation

About you

To be successful in this role, we are looking for someone with the following:

* A relevant qualification in a health-related field and/or significant experience working with clients in the delivery of community-based services
* Demonstrated knowledge and understanding of the impacts of dementia for individuals, carers and their support networks or the capacity to develop that knowledge base and understanding
* Demonstrated ability to engage and work in partnership with people living with dementia, and their carers
* Demonstrates sounds knowledge to navigate the use of information, database software in line with compliance and procedures, well-developed case notes and record keeping skills within the context of a client database
* Communicates clearly and openly with diverse audiences, promotes dialogue and consensus, and responds with active listening, respect, and understanding
* Collaborates with others to remove barriers and break down silos and enable the collaborative delivery of client/customer focused outcomes
* Uses effective time management skills to plan, evaluate and organise work demands to achieve priority outcomes
