About the Position
The Care Navigator role involves working collaboratively with consumers to connect them with community services and supports, facilitating person-centered and integrated care. This position delivers practical support through information sharing and education, identifying barriers to access, and facilitating warm referrals to ensure consumers receive appropriate care at the right time.
This role works closely with other members of the multidisciplinary team and external service providers to maximize care outcomes for consumers.
The successful candidate will have a strong understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
* Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
* Current driver's license and willingness to travel for the role.
