Job Description
We are seeking a compassionate and person-centred care professional to join our aged care facility in Port Macquarie. In this role, you will provide high-quality support to residents, fostering a warm and family-like atmosphere.
Key Responsibilities:
* Personal Care & Daily Living: Assist residents with personal hygiene, including showering, toileting, grooming, dressing, oral hygiene and skin care.
* Mobility: Support mobility and safe transfers using hoists or walking aids while encouraging independence.
* Mealtime & Nutrition Assistance: Aid residents during meals, including feeding support and monitoring dietary needs using appropriate adaptive methods.
* Support Health & Wellbeing: Support residents with chronic conditions, early palliative care needs or cognitive impairment such as dementia, while maintaining dignity and comfort.
* Clinical Support: Assist with medication administration, observe vital signs, report health concerns to nursing staff, and support basic health monitoring tasks under direction.
* Emotional & Social Connection: Provide companionship, meaningful conversations and emotional presence to reduce isolation and promote wellbeing.
* Documentation & Communication: Maintain accurate records of care provided, incidents, health changes, medication logs, resident preferences; communicate effectively with residents, families and healthcare professionals.
* Household & Environmental Support: Perform household duties to keep shared spaces safe and tidy.
* Safety & Compliance: Create a hazard-free environment and raise concerns regarding resident safety or wellbeing; comply with relevant codes, standards and confidentiality requirements.
* Team Collaboration: Work with Registered Nurses, Allied Health and Lifestyle coordinators to deliver holistic multidisciplinary care.
Requirements:
* Qualifications: Certificate III or IV in Individual Support (Ageing), or equivalent qualification.
* Experience: Previous experience in aged care or community care, or be actively pursuing a career in aged care; currently studying or recently qualified acceptable.
* Skills: Adaptable, empathetic and resourceful; alignment with values of being compassionate, respectful, imaginative and bold.
* Commitment: Commitment to a culturally safe and inclusive work environment.
* Ability: Ability to adapt to changing shifts if required.
* Passion: Genuine passion for elderly care, empathy, patience and strong communication skills.
