Our client a successful National Healthcare Solutions group have been providing on-site physiotherapy services to local communities for over a decade. Delivering cost effective Allied Health services for their clients they provide premium care experiences for aged care residents, empowering them to stay active, independent and safe from falls and to age with dignity.
Reporting to the Clinical Director you will work independently on site in the Port Macquarie region ( mid north - coast NSW) at a mid-sized aged care facility alongside experienced residential aged care staff to ensure an integrated care approach is delivered to all their residents.
Key duties of the role
* Assess residents' physical and functional abilities and limitations and develop customised physiotherapy programs, interventions and treatments to meet the client's health and wellness objectives and reduce health risks.
* Physiotherapy treatments and programs including group and one-on-one mobility, strength and conditioning and falls prevention exercise programs.
* Re-enablement and rehabilitation treatments for clients with functional or physical impairments resulting from injuries, falls or extended hospital stays
* Identification and treatment of muscular-skeletal imbalances, impairments and weaknesses that contribute to pain (including manual therapy treatments such as massage and joint mobilisation).
* Ensure residents are re-assessed on a regular basis to monitor effectiveness of the physiotherapy programs and make any necessary adjustments to treatments.
* Participate in falls review meetings and develop falls prevention strategies for residents.
* Assess the physical and functional capacity of residents and
ensure residents are re-assessed on a regular basis to ensure they remain correctly classified under AN-ACC
* Respond in a timely manner to all requests from aged care staff to assess and treat residents who have been injured or are at risk.
* Provide manual handling training and competency assessments .
* Ensure that all clinical notes are updated in a timely manner.
* Remain compliant with the Aged Care Quality Standards
Skills and requirements
Qualified physiotherapist with either AHPRA General Registration of Limited Registration Supervision Level Indirect 2 or Remote. Minimum 3 years' experience preferably in residential aged care or complex community settings.
Demonstrated ability to apply advanced clinical reasoning to assess residents' physical, functional, cognitive and environmental risk factors, and to translate assessments into clear, resident-specific physiotherapy recommendations.
Proven capability to provide clinical leadership in an aged care environment, including guiding care staff on mobility and risk management, and proactively recommending strategies to reduce falls.
High-level written and verbal communication skills, including the ability to communicate assessment outcomes, equipment changes, and risk-related recommendations clearly and promptly to multidisciplinary teams.
Ability to exercise professional judgement regarding what clinical information requires escalation to support resident safety and compliance.
Experience prescribing, fitting, signing off, and reviewing mobility and manual-handling equipment, with an understanding of professional accountability, resident safety, and ongoing review requirements.
Ability to adapt physiotherapy interventions for residents with cognitive impairment or behavioural challenges, including collaborating with care teams to revise strategies where residents resist treatment.
Strong organisational and time-management skills.
