Location: Cairns Central & Surrounding Areas | Employment Status: Permanent Part Time | Remuneration: $37.77 p/hr + 12% Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances & Travel Reimbursement (KM’s) + Employee discounts

About The Role
As a Senior Personal Carer you’ll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centred approach while providing quality care based on clients’ needs.
- Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
- Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team
- Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
- Providing care in accordance with their individualised Care Plan
- Supporting with community based recreational and social activities
- Transportation and assistance with shopping and appointments
Role Requirements
- Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
- Current Australian Driver’s Licence (Green Provisional minimum) - Essential
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work - Essential
- Experience with technology incl. smartphones, tablets, laptops etc
- Confidence to satisfy the relevant probity checks required by legislation or policies
