NDIS Support Coordinator – Contractor
Ace Navigation Pty Ltd is a small, director-led team of dedicated Support Coordinators supporting participants with complex needs across all Perth metropolitan areas. We are passionate about high-quality, ethical Support Coordination and take pride in making a genuine difference in participants' lives.
Due to demand and capacity constraints, we are seeking an experienced Support Coordinator contractor to take on a small initial caseload, with the opportunity to increase gradually.
This role is suited to a professional who can work independently, exercise sound judgment, and confidently manage complexity with minimal onboarding.

About the role
* Contractor role (ABN required)
* Small initial caseload with complex participants
* Primarily work-from-home, with face-to-face participant engagement as required and regular team catch-ups at local venues
* Flexible working hours by agreement (no guaranteed hours)
* Ongoing referrals available once capacity and fit are established

Essential requirements
* Minimum 12 months NDIS Support Coordination experience (non-negotiable)
* 2+ years' experience strongly preferred
* Demonstrated experience working with complex participants, either within Support Coordination or in a related field (e.g. mental health, hospital discharge, community services, disability case management)
* Strong understanding of NDIA processes, plan implementation and service agreements, and working with multiple stakeholders
* Ability to work independently with minimal supervision
* Excellent written documentation and professional communication skills
* Own ABN and appropriate insurances
* Current NDIS Worker Screening Check
* Current National Police Clearance
* Own laptop and ability to maintain appropriate cyber security
