You will play a crucial role in coordinating and facilitating the safe and efficient discharge of patients from our healthcare facility. Your primary responsibility will be to work closely with patients, their families, physicians, and other healthcare providers to create personalised discharge plans that address each patient's unique needs.
**Key Responsibilities**
- Conduct comprehensive assessments of patients' medical, social, and psychological needs prior to discharge.
- Collaborate with healthcare team members to develop individualised discharge plans that include appropriate post-discharge care and support services.
- Coordinate with external agencies and community resources to arrange for home care, rehabilitation, or other necessary services.
- Educate patients and their families about the discharge plan, including medication management, follow-up appointments, and any lifestyle adjustments needed for successful recovery.
- Address any potential barriers to successful discharge and develop strategies to overcome them.
- Participate in multidisciplinary rounds and meetings to provide input on discharge planning and progress updates.
- Maintain accurate and up-to-date documentation of all discharge planning activities.
- Continuously monitor and evaluate the effectiveness of discharge plans, making adjustments as needed.
**To be successful you will have the following skills/attributes.**
- Tertiary qualification in healthcare desirable
- Previous experience in discharge planning, care coordination, or case management within a healthcare setting.
- Strong understanding of medical terminology, treatment options, and post-acute care resources.
- Excellent communication and interpersonal skills to effectively work with patients, families, and healthcare professionals.
- Ability to prioritise and manage multiple cases while ensuring attention to detail.
- Familiarity with relevant regulations and insurance requirements related to discharge planning.
- Empathy, compassion, and a patient-centred approach to care.
