The Role
The Support Coordinator is the key representative of Black Swan Health providing specified individual services, coordinating a range of supports that address the needs of clients, supporting individuals in accessing and principally providing a range of psychosocial supports including recovery coaching.
Support Coordinators are responsible for supporting consumers through NDIS eligibility processes, planning for and development of individualised funding packages and importantly, provision of services and supports identified in individualised funding packages.
Support Coordinators apply a person-centred approach to service delivery that aligns with the implementation of the NDIS Quality and Safeguarding Framework and National Mental Health Standards.
Duties and Responsibilities of the Position
§ Provide Support Coordination and other support to clients with a NDIS Individualised Funding Package.
§ Provide Support Coordination and other supports to clients engaged in Black Swan Health psychosocial support programs.
§ Provide education, information and support for clients to assess eligibility for NDIS and have a NDIS individualised funding package developed.
§ Develop productive and meaningful relationships with clients, their families and carers and or other supports.
§ Develop, monitor and review NDIS Individualised Funding Plans and psychosocial support action plans for clients and maintain regular contact with the client to provide person-centred care.
§ Maintain a robust understanding of NDIS and impact to clients; and continually improve processes for supporting clients through NDIS processes.
§ Undertake triage and safety assessment of clients as required.
§ Timely submission of case notes, records and client information to enable responsive service management activity including NDIS invoicing and reporting.
§ Timely management of client information and records.
§ Conduct all activities in compliance with relevant legislation including Equal Employment Opportunity, Occupational Health and Safety, and all policies, procedures and guideline as determined by Black Swan Health.
§ Other duties as required.
The successful candidate will
· Certificate IV in relevant sector of support, e.g. Mental Health, Community Services, Disability Services.
· Knowledge of the National Disability Insurance Scheme.
· Experience in working in an outreach role.
· Experience in working with Aboriginal and Torres Strait Islander people and people from Culturally and Linguistically Diverse backgrounds.
· Experience using Microsoft Office and electronic Client Information Management Systems.
· Current National Police Clearance (no more than 6 months old)
· Working with Children's Check
· NDIS Worker Screening Check.
· Current driver's licence and own vehicle for work purposes
· Current eligibility to work in Australia
· Demonstrated experience, working with people who live with a disability and/or with severe and persistent mental illness.
· A demonstrated person-centred approach.
· Strong case and stakeholder management skills.
· Ability to work autonomously but as part of a multi-disciplinary team across diverse locations.
· Highly developed interpersonal communication skills including relationship building, negotiation and conflict resolution skills.
