We’re hiring: Elder Care Community Coordinators & Community Connectors
Organisation: Moorditj Koort Aboriginal Corporation (MKAC)
Locations:

Parmelia

- Midland

Positions: Coordinators (SCHADS L4) • Connectors (SCHADS L3)

Two great roles — pick your best fit

Option 1: Elder Care Community Coordinator (SCHADS Level 4)
Lead a small site team to keep work flowing smoothly and support quality care.

- Coach and support Connectors; run short weekly huddles and case reviews

- Build relationships with assessors and providers; organise case conferences as needed

- Keep data tight: same-day notes, simple workflows, on-time monthly reporting
Pay & benefits: SCHADS L4 + super + generous salary packaging (PBI)

Option 2: Elder Care Community Connector (SCHADS Level 3)
Walk alongside Elders and families to get support started quickly and safely.

- Explain options in plain language; support My Aged Care registrations/assessments

- Liaise with hospitals and aged-care providers; run yarning/info sessions

- Keep clear, accurate records that help us help more Elders
Pay & benefits: SCHADS L3 + super + generous salary packaging (PBI)

What you’ll bring

- Heart for community, cultural safety and great communication

- Organised, reliable and calm — you get things done and keep people informed

- Current WA C-class driver’s licence, National Police Check & Working With Children Check (or willing to obtain)

- COVID-19 Vaccination

Even better (nice-to-have)

- Experience in Aboriginal health, aged care, community services or care navigation

- For Coordinators: prior team lead/supervision or coaching experience
