We are seeking a compassionate, skilled, and clinically confident Registered Nurse to join our team on a permanent part‐time basis (15 hours per week), with the opportunity to increase hours as the business grows. Your Role
As an Case Manager/Care Partner, you will play a central role in delivering high‐quality, person‐centred care across DVA Community Nursing and My Aged Care programs. The role is based in Slacks Creek, with a travel requirement as needed from North Brisbane to the Gold Coast. Your responsibilities will include:
Managing a caseload and maintaining regular client contact
Conducting comprehensive nursing assessments utilising evidence‐based tools
Developing, implementing, and reviewing care plans in line with DVA Community Nursing and Support at Home programs
Coordinating services to ensure safe, responsive, and culturally appropriate care
Delivering Clinical Care to support clients in their home ensuring continuity of care
Supporting Support Workers and Enrolled Nurses with clinical guidance
Ensuring compliance with legislation, funding requirements, and organisational policies
Monitoring client budgets and escalating discrepancies
Contributing to quality improvement and community engagement.

Who We Are Looking For
Registered Nurse with current AHPRA registration
Experience in community nursing, aged care, or DVA programs
Strong clinical assessment, care planning skills and IT skills
Excellent communication and interpersonal abilities
Ability to work autonomously and manage a caseload effectively
Commitment to person‐centred, culturally safe care
Current driver's licence and reliable vehicle
Working with Children Check, Police Clearance, First Aid, CPR and Basic Life Support, or ability to obtain.
