We're currently seeking an experienced Clinical Case Manager (CCM) to join our dedicated Care Team.
This is a fantastic opportunity for a Registered Nurse who's ready to take the next step into a coordination role — combining clinical expertise with a genuine focus on person-centered care.
As a Clinical Case Manager, you will:
Coordinate and oversee client centered care planning to ensure services are appropriate, responsive and person-focused
Manage and monitor the delivery of in-home support services ensuring compliance to Aged Care Quality Standards
Build and maintain strong client relationships by providing ongoing support, communication and guidance throughout their care journey
Collaborate with multidisciplinary teams to document, review and ensure compliance with care plans and all related activities
To succeed in this role, you'll bring:
Current AHPRA registration as a Registered Nurse.
Experience in Home Care Packages, community, or aged care settings.
Strong communication skills, with a hands-on approach.
Excellent time management and problem-solving abilities.
A genuine passion for supporting older Australians to live their best lives
