Home Health Service Coordinator Job Summary
We are seeking an experienced Home Health Service Coordinator to lead our team of Care Workers in delivering exceptional client care services. The successful candidate will be responsible for managing a portfolio of clients receiving In-Home care and leading a team of professionals to provide tailored services and support.
Key Responsibilities:
* Manage and lead a team of Care Workers, including operational leadership and staff development.
* Assist the Branch Manager with overall leadership support for the area, staff, and clients.
* Ensure excellence in client service delivery, including client care assessments, goal planning, and onboarding new clients.
* Manage client escalations and complaints to a high level of resolution.
* Develop strong relationships with clients and their families, and build effective collaborative relationships with other relevant service providers.
Requirements:
* Previous experience managing a service area or program within a health care or community service organization.
* Strong people leadership experience, developing and maintaining high-level team performance.
* Experience managing remote direct reports and a dispersed workforce would be beneficial.
* Proven experience in coaching and developing individual and team performance.
* Experience managing a high-volume client caseload, including complex client care needs, service allocation, compliance, and case management.
