Job Responsibilities
* Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate.
* Document evaluations, treatments, and outcomes in the medical record.
* Work 32 hours per week with 8-hour shifts.
* Collaborate with the care team to coordinate therapy plans and discharge recommendations.
* Provide therapeutic interventions and track patient progress toward measurable goals.
Qualifications
* Must have 2+ years of experience.
