CNS2, Chronic Care Disease Management - Clarence

Temporary Part Time position within the Northern NSW Local Health District (NNSWLHD) focused on chronic disease care coordination and case management.

About the Role

Clarence Community & Allied Health have a temporary vacancy for a Clinical Nurse Specialist to join their team to provide care coordination, case management and support to people with chronic disease, their carers, General Practitioners, practice staff and staff involved in the provision of care. Working with relevant stakeholders to ensure coordination of services, utilising shared care plans and other team care approaches to optimally support people with chronic disease within the framework of the Chronic Disease Management Service.

Responsibilities

- Provide care coordination, case management and support to people with chronic disease, their carers, General Practitioners and practice staff.
- Collaborate with stakeholders to coordinate services and utilize shared care plans and team care approaches.
