Overview
This is an opportunity for an enthusiastic, motivated Social Worker to join a flexible and dynamic team providing psycho-social support, family and carer liaison, counselling, discharge planning and patient education. You will work independently with patients and as a multidisciplinary team member to provide quality services to both inpatients and day patients. Experience in rehabilitation services would be beneficial.
Responsibilities
* Provide psycho-social support, family and carer liaison, counselling, discharge planning and patient education to inpatients and day patients.
* Work independently with patients and as part of a multidisciplinary team to deliver quality services.
* Engage with rehabilitation programs across orthopaedic, neurological, cardiac, respiratory and oncology rehabilitation as part of the multidisciplinary team.
Qualifications
* Appropriate tertiary qualifications in Social Work
* Eligibility for membership of the Australian Association of Social Workers
* Ability to work autonomously
* Well-developed communication and interpersonal skills
* Excellent organisation, time management and self-management skills
* Commitment to ongoing education and the provision of evidence-based practice
