About the Role
At ConneXtions, our Support Workers direct participant support in line with our Values and Vision, by walking alongside participants in their individual journeys, empowering and supporting capacity building outcomes to assist participants to reach personal goals and outcomes, whilst ensuring their active participation as valued member of the communities in which they live.
We are currently experiencing an exciting period of growth and change, and we are seeking experienced, qualified, direct support workers to work with our NDIS, TAC and Aged Care participants.
Day to day responsibilities and duties of the position may include:
* Engaging in social & recreational activities to help develop meaningful relationships and social networks
* Assisting with pathways to education and employment, including job searching and workplace engagement
* Assisting participants in building essential life skills, such as cooking, cleaning, meal planning and budgeting
* Travel to appointments and navigating public transport
* Personal care, including use of hoists/lifting equipment
* Medication management
* Implementation of Individual Support Plans
* Completion of accurate progress notes and reports
About You

We are looking for people that have a minimum Certificate III Individual Support or Certificate IV in Disability and a willingness to complete training as requested.
You must be available to do a variety of shifts, including early mornings, overnight and awake shifts
You will be driven by a desire to help our participants achieve their goals and be relatable to the people we are supporting.
You will be passionate about having a positive impact and supporting people with disabilities to reach their full potential.
As a condition of employment, you are required to obtain and maintain compliance with:
* NDIS Worker Screening Check
* NDIS Worker Orientation Modules
* Working With Children Check (VIC & NSW)
* Current First Aid Certificate including CPR.
* Current driver's licence & access to a registered vehicle with Comprehensive Insurance
* Mandatory online training
