Conscious Healthcare SA is a boutique allied health service provider, driven by the belief that small acts of support, spark big waves of change in our community. Operating across South Australia we pride ourselves on our accessibility and tailor our services to individuals from a range of pathways including Medicare, private and NDIS. Our team blends evidence-based practice with trauma-informed and holistic approaches across psychology, OT, PBS, physiotherapy, dietetics, and speech pathology disciplines.

We are currently expanding our services and looking to grow our PBS capability with a newly created Positive Behaviour Support Practitioner role, based at our Seacliff or Salisbury clinics. This role cross collaborates with our allied health professional team to support our adult, adolescent and child participants in experiencing greater levels of confidence, independence and joy in their lives.

The Position:

* Deliver evidence-based PBS services to clients across NDIS, Medicare and private funding streams in a variety of settings (clinic, home, school, workplace, community).
* Support participants to identify and achieve meaningful goals, promoting independence, inclusion, and overall wellbeing.
* Conduct high-quality functional behaviour assessments and develop behaviour support plans that reflect best practice and comply with professional, ethical, and legislative requirements.
* Provide education, coaching, and capacity-building to families, carers, support workers, and other members of the participant's support network.
* Maintain strong stakeholder relationships with support coordinators, GPs, schools, and other community partners.
* Complete risk assessments and develop proactive, person-centred strategies to reduce behaviours of concern and minimise the use of restrictive practices.
* Support early intervention and preventative approaches, particularly for children and young people.
* Share knowledge and contribute positively to a continuous improvement workplace culture.

Requirements:

* Relevant tertiary qualifications in Psychology, Social Work, Occupational Therapy, Speech Pathology, or a related allied health/human services discipline.
* Current registration with the NDIS Commission, ensuring adherence to NDIS Quality and Safeguarding Framework and professional standards.
* Knowledge of disability, mental health, and developmental conditions, and their impact on behaviour and daily life.
* Demonstrated understanding of Positive Behaviour Support principles, evidence-based practice, and clinical reasoning skills.
* Excellent interpersonal, written, and verbal communication skills, with the ability to build effective relationships with participants, families, and support networks.
* Confidence in using IT systems and digital service delivery methods (e.g., telehealth).
* Current Working with Children Check and NDIS Worker Check.
* Current driver's licence and access to a reliable vehicle.
