We're partnering with a well‐established aged care provider to recruit a Clinical Care Coordinator for a coastal facility in this highly sought‐after Western Australian location.
The Opportunity
* Full-time, Monday to Friday
* Supportive leadership structure, reporting into an experienced Clinical Manager
* Work alongside another CCC in a collaborative environment
* Potential Relocation support + sponsorship available for candidates within Australia
The Role
You'll play a key role in leading clinical care delivery and supporting a high‐performing team, with a strong focus on quality, compliance, and resident outcomes.
* Oversee clinical care coordination and delivery
* Support and mentor nursing and care teams
* Ensure compliance with Aged Care Quality Standards
* Act as a key point of contact for families and stakeholders
* Contribute to audits, quality improvement, and clinical governance
About You
* AHPRA Registered Nurse
* 2+ years' experience in a clinical leadership role
* Strong understanding of aged care standards & compliance
* Confident leading, mentoring, and supporting teams
* Experience with audits, accreditation & continuous improvement
* Strong communication and stakeholder engagement skills
