Discharge Support Coordinator
We are seeking a skilled professional to join our Primary and Preventative Health team in the Discharge Support Service department for a fixed term period relief.
* This role involves working across acute and community services, aiming to achieve a smooth transition for clients/carers referred to the Discharge Support Service with complex discharge needs.
* The successful candidate will facilitate appropriate support services for clients referred to the Discharge Support Service and act as a broker in the purchase of those services.
* To be successful in this position, strong teamwork is required with multidisciplinary team members and external service providers.
* You will bring advanced clinical skills, effective contemporary nursing practices, capable and motivated registered nursing expertise, high levels of energy and ability to work independently and as part of a multidisciplinary team.
Key Responsibilities:
* Facilitate client discharges from hospital care back into their communities by coordinating necessary support services and resources.
* Provide ongoing support to clients and their carers through home visits, phone calls, or other forms of communication.
* Collaborate with healthcare professionals, social workers, and other stakeholders to ensure seamless transitions and optimal outcomes.
* Develop and implement personalized care plans that cater to individual client needs and preferences.
Requirements:
* Registered nurse qualification with current registration.
* Advanced clinical skills and experience in gerontology, pediatrics, or mental health.
* Excellent communication and interpersonal skills with the ability to build rapport with diverse clients and stakeholders.
* Ability to work independently and collaboratively within a dynamic team environment.
* High level of motivation, energy, and adaptability with a focus on delivering exceptional patient outcomes.
