Together we can do more. Provide high-quality social work services to patients, clients, consumers, and their families/carers to ensure optimal health outcomes, in accordance with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
This position specifically serves the Shoalhaven Aged Care Assessment Team (ACAT).
The key responsibilities include conducting comprehensive aged care assessments, providing referrals, support, and information regarding services that require access to Commonwealth-funded aged care placements or alternative services.
Formulating care plans to meet the needs of patients/clients/consumers according to the Department of Health and Aging and adhering to ACAT Guidelines.
Recent aged care experience would be advantageous.
The role may require work at other ISLHD sites.
