We are excited to offer this opportunity to join our Specialist Community Palliative Care team who deliver evidence-based quality care to individuals and their families/caregivers on the Bellarine Peninsula. About the Team
The BCH Palliative Care service comprises an interdisciplinary team of Specialist Community Palliative Care Nurses and a Family Support Worker. We work in conjunction with BCH Community Nurses and Occupational Therapists with access to Specialist Medical support.

About the role
The Community Palliative Care Nurse will provide holistic, evidence-based quality care for the individual with a life-limiting illness and their families/caregivers, in the community, within an interdisciplinary care model. The primary aim of the position is to improve patient outcomes, access to clinical services, and enhance the care of the individual requiring palliative and end of life care.
We would like to hear from you, if you...
* Are a Registered Nurse (with current registration with Nursing and Midwifery Board of Australia).
* Are experienced in community Palliative Care (essential).
* Hold Post Graduate qualifications in Palliative Care or Oncology (or working towards in agreed time frame).
* Can demonstrate leadership in the knowledge, philosophy, principles, and practice of Palliative Care.
* Are able to provide professional, accurate and evidence based Palliative Care support and advice to all stakeholders.
