The role involves assessing client needs, planning and implementing care plans, supporting community and acute nursing staff, and coordinating service delivery to ensure patients and carers receive the right care and support.
This position requires advanced wound care knowledge and skills to meet the complex care needs of individuals across the continuum of care.
You will work with a high level of autonomy, delivering specialised services closer to home for clients across the region.
Managing a defined caseload, you will provide care at a competent to proficient level, consolidated by your additional training and experience.
While the role is predominantly clinical, you will also serve as a valuable professional resource, providing advanced knowledge and support for consumers living with a stoma.
We are looking for a Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia, who holds or is eligible to hold a current practicing certificate, relevant postgraduate qualifications or working towards in stoma therapy nursing or equivalent, and registered nurse/midwife with at least 3 years full-time equivalent post-registration experience with stomal therapy.
