WhiteparishSurgery is looking to appoint an experienced and motivated Practice Nurse to join our established clinical team, following the retirement of a long standing member of staff. This is an excellent opportunity to become part of a well regarded rural dispensing practice serving a growing population of approximately 7,800 patients.
The successful candidate will provide a wide range of nursing services, including treatment room duties, immunisations, wound care, cervical screening and chronic disease management. A commitment to high standards of clinical practice and excellent communication skills are essential.
The role is for 20-24 hours per week, worked over 3 days. Specific working days can be discussed and agreed with the successful candidate to support both service needs and individual preferences. Salary will be dependent on qualifications and experience.
Informal visits to the Practice are welcomed by prior arrangement.
Main duties of the job

* Contraception

* Enable individuals with long term conditions and their carers to make informed choices concerning their health and well being and to organise their own support, assistance and action and promoting self care
* Support individuals with long term conditions and their carers to change their behaviour and to reduce the risk of complications
* Manage programmes of care for patients with acute and chronic disease by planning and evaluating care
* Work across boundaries and in partnership with primary and secondary care clinicians and social services, coordinating care promoting a multi-disciplinary approach.
* Review prescribed medication and provide advice on all aspects of medicines management, within scope of practice. Refer to relevant clinicians where appropriate.
Treatment room practice:
* Complex wound management
* Vaccinations and immunisations
* Infection, prevention and control
* Assess, plan, provide and evaluate nursing care to meet the needs of individuals and groups in the practice population.
