Occupational Therapist – Community & Home-Based Services

Location: Hervey Bay, QLD

Employment Type: Full-time, Permanent


Position Purpose

The Occupational Therapist is responsible for delivering high-quality, client-centred occupational therapy services across home, community, and clinic environments. This role supports clients to enhance independence, safety, functional capacity, and overall quality of life through evidence-based assessment and intervention.


Key Responsibilities


Clinical Service Delivery

* Conduct comprehensive functional assessments, including ADLs, home environment, cognitive/functional capacity, and equipment needs.
* Provide evidence-based therapeutic interventions across paediatric, adult, and ageing populations depending on caseload.
* Deliver home and community-based therapy that builds capacity, strengthens independence, and promotes meaningful participation.
* Recommend, trial and prescribe Assistive Technology (AT) in line with relevant standards and NDIS requirements.
* Assess and recommend minor and complex home modifications, working with builders and relevant specialists as required.
* Deliver neuro-rehabilitation services (e.g., stroke, ABI, CP, Parkinson's, MS, and progressive neurological conditions).
* Support clients across a variety of funding streams including NDIS, Home Care Packages, DVA, private health and community programs.
* Develop personalised intervention plans, set functional goals and monitor progress.


Documentation & Reporting

* Prepare professional clinical reports including functional assessments, AT prescription reports, home modification assessments, progress reviews, and discharge summaries.
* Ensure documentation is completed within required timeframes and meets NDIS and organisational standards.
* Maintain accurate clinical records in line with privacy, legislation, and professional requirements.
* Work collaboratively with multidisciplinary team members, families, carers, support coordinators, and external stakeholders.
* Consult with GPs, allied health professionals, support workers and service providers to ensure coordinated care.
* Advocate for clients to ensure their goals, needs and safety are prioritised.


Professional Standards & Compliance

* Uphold AHPRA standards, the OT Board Code of Conduct, and all safety and quality frameworks.
* Maintain compliance with legislative requirements including NDIS Practice Standards, Manual Handling, Infection Control and Mandatory Reporting obligations.
* Engage in reflective practice, supervision and ongoing professional development.


Qualifications & Experience

* Bachelor or Master of Occupational Therapy.
* Current AHPRA registration.
* Valid driver's licence
* National Police Check and Working with Children Check (or ability to obtain).
* Strong verbal, written and interpersonal communication skills.
* Experience in community OT, NDIS, home modifications, or assistive technology.
* Knowledge of neuro-rehabilitation and functional capacity building.
* Previous experience working autonomously in a community-based role.
