About the Role: Previous experience working as a Practice Nurse in a Family Medical Clinic is essential. We are an expanding practice that requires an experienced Practice Nurse who enjoys Chronic Disease Management and has Immunisation certification. You will work in a team which supports GP's and their patients to optimise their chronic conditions. Essential Skills: Ensure all patients and visitors are received in a friendly and helpful manner, either in person or over the phone. Enjoys Chronic Care Nursing. Immunisation Certificate must be held at the time of application Has the clinical skills to conduct detailed health assessments to improve patient health outcomes Identify, assess and develop chronic disease management care plans that optimises improved health outcomes Has a passion for managing, educating and empowering patients on preventative health care. Promote preventive screening Best Practice software experience is preferred, with training provided for successful candidates Maintaining demographic and clinical data in patient files often using software like Polar Explorer. Maintain a high level of accuracy when recording and maintaining patient and clinical information Identify and provide physical and emotional patient care in your role Adhere to RACGP 5th Edition Standards and AGPAL Accreditation requirements Additional skills: Spirometry - trained Ear syringe - trained Phlebotomy - trained Wound care Maintain Infection Control standards Cold Chain Management Essential Requirements: Minimum 2 years of Practice Nurse experience in a Family Medical Clinic is required.
