Remuneration: $131,870.90 - $134,500.69 per annum
Hours Per Week: 38 (Job Share may be considered for part time hours)

What You'll Be Doing
* The Pathways to Community Living Initiative (PCLI) is a state-wide coordinated approach to supporting people with enduring and serious mental illness who have been in hospital more than twelve months and or are likely to experience difficulty with discharge pathways, to re-establish their lives in the community. PCLI was established to support the transition of long-stay mental health patients into appropriate community- based living arrangements and services. Western Sydney Local Health District Mental Health Service is committed to building a professional workforce that supports consumers with severe and persistent mental illness on their recovery journey.
* The role of the PCLI CNC is to provide evidence-based care that supports safe transitions of consumers into care in a community setting (or inpatient setting) based on the individual consumers' strengths, needs and wishes. These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.
* A primary goal of this position is to work collaboratively with existing inpatient and community teams, consumers, carers and other key stakeholders in the coordination of day-to-day work practices for teams working with long-stay consumers, by supporting the assessment and progression of safe transitions of consumers into care in a community setting while promoting effective participation in mental health treatment, support and recovery. This role will carry a clinical caseload, championing practice improvement through mentorship and supervision, to ensure the delivery of evidence-based, strengths focused, recovery orientated assessment and treatment.

Essential Requirements
* Qualification: Master's degree in nursing or equivalent
* Registration: Current registered Nurse with the Australian health Practitioner Regulation Agency (AHPRA)
Selection Criteria
* Five (5) years' experience post tertiary qualification in mental health with at least 3 years' experience working in age care mental health nursing.
* Demonstrated skills in mental health and discipline specific evidence based, strengths focused, recovery orientated assessments and interventions.
* Demonstrated commitment to the philosophy and practice of consumer's families and natural supports participating within the treatment process, as appropriate and in accordance with the NSW Mental Health Act 2007.
* Demonstrated capacity to function at an advanced clinical level and to provide leadership and clinical Supervision, within a multi-disciplinary treatment setting.
* Effective interpersonal skills, problem solving skills and personal resilience with demonstrated ability to develop and maintain effective partnerships with a range of key stakeholders including, GPs, nongovernment organisations, inpatient and community mental health teams, consumers and carers.
* Highly developed written and verbal communication skills and competence in the use of information Technology systems.
* Ability to prioritise competing demands and work independently in consultation with the multidisciplinary teams across the district.
* Current unrestricted NSW driver's license and willingness to use same for work purposes.
