The** Transition Specialist**: Transition Specialist position is to provide support to NDIS participants and their families during the transition process into Specialist Disability Accommodation (SDA). Work closely with NDIS participants, their families, and support coordinators the role will develop transition plans, provide education and information about SDA options, and ensure a smooth and seamless transition into Specialist Disability Accommodation (SDA).
**Empowerment through Collaboration: Duties and responsibilities will include, but not limited to**:
- ** P**articipant Liaison**: Nurture connections with Participants, Guardians, and Service Providers, steering the path towards ideal accommodations.
- ** SIL Synergy**: Collaborate with SIL providers, seamlessly weaving Participants into their current homes.
- ** Funding Facilitation**: Ensure the perfect match between participants and funding, guaranteeing uninterrupted SDA access.
- ** Occupancy Oversight**: Skilfully manage accommodation occupancy, enriching lives through comfortable living spaces.
- ** Data collection**: Maintain crucial SDA documents, agreements, and leases, fostering structured environments.
- ** Code of Conduct Ambassador**: Embrace our values and champion the safety and wellbeing of all.
- ** Time Management Maestro**: Thrive within deadlines, executing tasks with skill.
- ** Customer Care Champion**: exceptional service, cultivating bonds with stakeholders, with commitment to the care, safety and wellbeing of yourself, participants and others around you.
*** Qualifications that Command Respect**
- 3+ Years Expertise: Savvy in tenancy, vacancy, or property management realms.
- NDIS Navigator: Navigated the NDIS and SDA sector, steering lives toward better tomorrows.
- Deep knowledge and understanding of SDA operations, how it operations, the people it benefits with an understanding of SDA contributions and association funds.
- NDIS Connoisseur: In-depth grasp of the National Disability Insurance Scheme.
- Property Prowess: exposure and general understanding of property operations
- Communications Extraordinaire: Forge unbreakable bonds with stakeholders through stellar communication.
