**What You Will Be Doing**
The Community Care Social Worker is responsible for coordinating intensive short term (3-6 months) outreach care and delivering allied health interventions within their scope of practice to clients who have chronic health conditions and / or complex health needs.
The role involves facilitating collaborative integrated care planning across acute and community based services. Care coordinators support people to navigate the service system and services. They provide the point of contact as people undertake their care plan.
**Key Responsibilities**
- Provide safe, cost effective outreach assessment and multi disciplinary care planning within a social model of health considering the impact on positive health outcomes.
- Understand chronic disease processes and their impact on the client/ carer.
- Understand the concept of health literacy, the individual level of health literacy and the impact this has on the client ability to manage their care.
- Provide allied health services including thorough assessment, intervention and discharge planning within scope of practice.
- Demonstrate current evidence based practice within your area of clinical expertise.
- Ability to work with clients on transition to other services once goals are complete.
**What You Need**
We anticipate you will have:
- Qualification in Social Work.
- Current Victorian drivers license.
- Demonstrated experience working with at risk and vulnerable people with complex needs
- Demonstrated knowledge and experience with chronic disease management.
Ability to manage multiple competing tasks and effectively prioritize.
