About us:
Community Lifestyles Agency is a not for profit organisation that has been providing supports to people with disabilities and their families on the Fraser Coast for over 35 years.
The Supports Facilitator will lead a team of dedicated disability support workers to provide exceptional support and services to people with disability on the Fraser Coast.
About the role:
The role includes developing strong and long lasting relationships with the people receiving supports, as well as all team members, to ensure exceptional services are provided, providing on the job training and mentoring for team members, responding to and investigating incidents, and being part of a rotating On Call team for afterhours supports.
About you:
* Experience in the disability/community services sector
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Ability to deliver services that respect and respond to individual needs and preferences
* Relevant qualifications or equivalent experience
Role Responsibility:
* Caseload management of allocated service users, relevant staff and team leaders;
* Roster and delegation of shifts/relevant HR process/pay disputes;
* Writing lifestyle plans/education transitions;
* Foster 'independence' concepts and service delivery with staff, service users and community;
* Deliver Training/ education opportunities to both members of the sector/families and friends of service users/service users themselves;
* Enhance opportunities for community inclusion through networking;
* Enhance funding opportunities with networking within community;
* Identify potential areas of improvement and implement training;
* Partake in development opportunities personally and forecast future business direction relevant to sector;
* Organising and participating in group activities and overnight trips and opportunities; and
* Social Media presence and marketing of organisation, creating positive interactions with service users and families. Support with Cyber presence behaviours for service users to ensure safety as possible.
Selection Criteria:
* Through leadership and role modelling ensure that the support provided promotes the dignity, independence and the rights of each person;
* Foster the concept of ‘independence’ not dependency;
* Establish and maintain effective lines of communication with parents, families, carers, and advocates and other service providers. In doing this, ensure that a co-operative atmosphere of mutual respect, confidence and trust exists;
* Using a team approach, ensure you are aware and understand the need to respect, foster and support each individual's natural networks and links within their community; and
* Work towards breaking down any negative community values which could infringe on a person's quality of life.
Qualifications & Requirements
* Relevant Tertiary Qualification (desirable)
* Current QLD Driver’s License
* Willingness or ability to obtain:
* Blue Card (Working with Children)
* NDIS Worker Screening Check
