This is an exciting opportunity for a Pharmacy Technician to join South Lincoln Healthcare as part of our Clinical Pharmacy Team.

The post holder will work with our existing Clinical Pharmacy Team across our practices (Bassingham Surgery, Branston & Heighington Family Practice, Brant Road & Springcliffe Surgery, Church Walk Surgery, Cliff Villages Medical Practice, The Heath Surgery, Washingborough Family Practice).

The role will involve a close working relationship with our GP practices, innovative HomeHealth, and Living Well Teams.


Main duties of the job

The role will support Clinical Pharmacists to ensure the delivery of safe, effective, and efficient systems for repeat prescribing, medicines optimisation, waste reduction, and maximising patient outcomes, including:

* Responding to medicines-related queries & requests
* Reconciliation of medicines post-discharge from hospital, intermediate care, & into care homes, including identifying & rectifying unexplained changes & working with patients, admissions & discharge care coordinators, and community pharmacists to ensure patients receive the medicines they need
* Reconciliation of medication following specialist/secondary care contacts/advice
* Reviewing the use of medicines commonly associated with unplanned hospital admissions through audit & patient review
* Working with clinicians/CPs to develop and implement robust systems for shared care prescribing, high-risk drug monitoring & drug monitoring for optimisation of LTCs
* Having their own clinic after suitable training & competencies assessment eg BPreview

Job responsibilities

Clinical Responsibilities

* Undertake patient-facing and supporting roles to ensure effective medicines use, through shared decision-making conversations with patients
* Carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews, and medicines reconciliation. Use consultation skills to work in partnership with patients to ensure effective use of medicines
* Provide primary support to general practice staff regarding prescription and medication queries
* Support medicines reconciliation on transfer of care and systems for safer prescribing
* Support medication reviews and reconciliation for new care home patients and synchronise medicines for patient transfers, linking with community pharmacists
* Provide expertise to address public health and social care needs, including lifestyle advice and tackling health inequalities
* Take a central role in shared care protocols and liaise with specialist pharmacists for complex patients
* Support antimicrobial stewardship initiatives to reduce inappropriate antibiotic prescribing
* Assist in medicines optimisation and management schemes and patient safety audits
* Support the implementation of national prescribing policies and guidance within GP practices, care homes, and other primary care settings

Administrative responsibilities

* Work with the PCN multidisciplinary team to ensure efficient medicines optimisation, including ordering, return processes, and reducing wastage
* Supervise practice reception teams in processing prescription requests
* Lead medicines optimisation systems across PCNs, supporting practices with electronic prescriptions, safe repeat prescribing, and high-risk medicine monitoring
* Provide training on legal, safe, and secure handling of medicines, including EPS
* Build relationships with pharmacy technicians, pharmacists, and other team members to support integrated care

The post holder will:

* Manage personal workloads effectively
* Follow departmental policies and guidelines
* Participate in clinical supervision, training, and service redesign
* Promote patient safety through incident reporting and investigations
* Signpost patients to appropriate healthcare professionals
* Maintain accurate clinical records
* Be qualified or willing to undertake the CPPE Pathway
* Review guidance to ensure compliance with NICE, CQC, etc.
* Support delivery of enhanced services and participate in patient complaints management
* Complete mandatory training and contribute to clinical governance
* Attend annual appraisals and set performance objectives
* Contribute to public health campaigns and quality initiatives
* Perform other tasks as assigned


Person Specification


Experience

* Qualified, registered pharmacy technician in primary care, community, or hospital pharmacy

Qualifications

* GPhC registration
* BTEC/NVQ Level 3 or equivalent in pharmaceutical sciences
* CPPE pathway diploma or willingness to undertake
* Evidence of CPD


Skills and Knowledge

* Knowledge of evidence-based medicine, pharmacy law, and ethics
* Proficiency in Microsoft Office and SystmOne
* Understanding of healthcare policies, prescribing data analysis, and primary care systems
* Strong communication, negotiation, and data confidentiality skills
* Knowledge of NHS Long Term Plan and PCN operations
