Care Manager Role
As a Care Manager, you will lead home clinical teams in delivering exceptional person-centred care. You will work closely with nurses, carers, residents, and families to create a positive and inclusive environment where staff can grow and thrive.
Key Responsibilities:
* Lead the clinical team to ensure high-quality resident care, including nurses, care staff, and allied health professionals.
* Develop and implement person-centred care plans tailored to individual residents' needs.
* Maintain full compliance with Aged Care Quality Standards, internal policies, and relevant legislation.
* Monitor clinical performance, conduct audits, and implement quality improvement initiatives to maintain high standards.
* Mentor, coach, and support staff development to foster a positive and collaborative workplace culture.
* Liaise with residents, families, and healthcare providers to maintain strong communication and address concerns promptly.
* Oversee medication management, infection prevention and control, and clinical risk management practices.
* Participate in the recruitment, induction, and ongoing training of clinical staff to ensure they have the necessary skills and knowledge.
* Prepare and submit reports for executive management, accreditation, and regulatory bodies to demonstrate accountability and transparency.
