We are seeking applications from enthusiastic and experienced Clinical Nurses to provide high-quality outreach visiting services to patients in residential aged care facilities, in a supportive and collaborative team environment.
This vacancy is for Permanent Full Time, Permanent Part Time, Fixed Term Full Time, and Fixed Term Part Time appointments. Fixed Term appointments may be extended and/or become permanent.
What is our Clinical Nurse – Residential Care Line role?
The Residential Care Line Service, based at Sir Charles Gairdner Hospital (SCGH), is Western Australia's only statewide, nurse-led outreach service providing support to residential aged care staff to deliver care in place.
Our passionate team of nurses collaborates with Residential Aged Care Facility (RACF) staff and practitioners, providing clinical excellence, expert knowledge, and education to empower RACF staff to improve the quality of life for older people living in residential care, supporting their choice to receive care in their 'own home'.
We are seeking enthusiastic and innovative Clinical Nurses, from primary care or hospital settings with aged care expertise, to join our team.
The position, under the direction of the relevant Senior Registered Nurse(s), practices as a Clinical Nurse in accordance with the Australian Nursing and Midwifery Board Registration Standards and Nursing Practice Decision Making Flowchart. The Clinical Nurse provides advanced, comprehensive, and evidence-based nursing care as part of a multidisciplinary team, acting as a role model, providing leadership, support, team building, and change management through respect, recognition, and collaboration.
Applicants will be required to support a roster that operates from 0800-1800, Monday to Sunday.
