Job Title: Support Worker
We are seeking a skilled and compassionate Support Worker to join our dynamic team. As a casual position, you will work with individuals living with disability or older Australians in their community.
Your primary responsibility will be to support customers in achieving their goals, promoting independence, and enhancing their overall wellbeing.

* They will also have the ability to develop individualised person-centred support plans that incorporate customer wellbeing needs, strengths, and goals.
Responsibilities:
* Collaborate with customers to develop support plans tailored to their unique needs.
* Provide support to improve independence, enabling customers to live and recover within their community.
* Assist customers with skill development, transport, social activities, finances, appointments, cooking, medication, and personal care.
* Maintain accurate customer records using online systems.
* Develop professional relationships and networks with customers, families, caregivers, and stakeholders.
* Support customers in accessing additional services to enhance their recovery journeys and overall experiences.
About You
* A minimum Cert III in Disability, Mental Health, Peer Work, Individual Support, or equivalent is required.
* A valid NSW Driver's License (Class C) with the ability to drive large cars and/or vans is necessary.
* An Australian Police Check, NSW WWCC, and NDISWC or willingness to obtain before commencing the role is expected.
