About the Role
We are seeking an organised, proactive Service Coordinator to join our team. This hybrid position offers a mix of working from home and attending our North-West Tas office and participant/team meetings when required.
This is a key operational role responsible for coordinating disability supports, ensuring service quality, overseeing documentation, and maintaining compliance with NDIS and organisational standards.
Importantly, the Service Coordinator also works closely with, directs, and supports a Document Compliance Officer, who assists with all documentation, record-keeping and compliance processes associated with service coordination.
Key Responsibilities
* Coordinate and monitor support services in line with participant plans and NDIS requirements
* Oversee day-to-day workflow, rostering, operational priorities, and service quality
* Maintain accurate documentation and ensure compliance with NDIS and organisational requirements
* Work with and provide direction to the Document Compliance Officer regarding documentation, records, and compliance tasks
* Liaise with participants, families, support workers, and external stakeholders
* Review and update forms, processes, service records and documentation
* Conduct compliance checks and contribute to continuous improvement
* Manage service-related communications and follow up actions
* Collaborate with internal teams to ensure smooth service delivery
* Participate in rotating on‑call roster
What We're Looking For
* Experience in disability services, community services, case management, or service coordination
* Knowledge of NDIS service delivery, documentation, and compliance
* Strong organisational and time-management skills
* Excellent communication and stakeholder engagement abilities
* High attention to detail and ability to manage competing priorities
* Confidence working independently and within a team environment
* Must live in North-West Tasmania and be available for in-office days and local meetings
* Certificate III/IV in Disability or Community Services (desirable)
* Current WWVP, First Aid/CPR, Manual Handling, Police Check (<3yrs), Driver's Licence
