About The Role
As a Registered Nurse in the HITH team, you'll play a vital role in supporting patients post-hospitalisation. You will work collaboratively within a multi-disciplinary team to provide a wide range of care including:
* Comprehensive nursing assessments
* Clinical observations and medication administration
* Wound care and pathology collection
* Client education and advocacy
* Medication reconciliation
* Liaison with HITH medical officers and other health professionals
About You
We are looking for a dedicated Registered Nurse with:
* Extensive clinical experience and strong decision-making skills
* Excellent communication and listening skills
* The ability to work autonomously while managing competing priorities
* Confidence in assessing and responding to patient needs in the community setting
