POSITION DETAILS
The Case Worker is a key member of the Wide Bay Community Resilience Program, a grant‑funded initiative delivered in partnership with the Country to Coast Primary Health Network. The role provides community‑based intake and assessment for vulnerable older adults, focusing on their physical, environmental, and social needs, and facilitates appropriate referrals for ongoing support and care. The position reports to the Manager, Aged Care Services, and provides day‑to‑day supervision, guidance, and support to the program's Specialist Support Workers to ensure high‑quality and coordinated service delivery.
KEY RESPONSIBILITIES
* Assess vulnerable older‑adult physical, environmental, and social circumstances in the community, with respect to disaster preparedness
* Support clients by identifying and actioning any of three referral streams available for ongoing Specialist Support Worker assistance (Strength and Mobility support, Environmental Management, and/or Social Connection), where required.
* Liaise with Intake and Referral Officer to optimise timely referral management, client journey and discharge, and facilitation of program data collection and reporting
* Liaise with client stakeholders and referrers to provide a high‑quality level of multidisciplinary care
* Provide support and mentorship to Specialist Support Workers to deliver their respective program streams
* Promote and present to relevant partners regarding program awareness, scope, and outcomes (PHN, GP practices, QAS, etc.)
* Engage and develop relationships with relevant volunteer organisations
* In collaboration with Manager Aged Care Services, contribute towards developing a feasibility study to highlight program effectiveness
* Schedule and cluster appointment times across the Bundaberg and Fraser Coast regions, to optimise time and cost efficiency
* Provide supervision, guidance and direction to the program team to ensure that program deliverables are met, and client care is optimised.
* Ensure accurate and timely record keeping required for program reporting purposes
* Participate when required in internal and external audit and quality assurance activities, with respect to continuous improvement of exercise physiology service provision
* Under direction of ACS leadership team, participate in project or program development activities to ensure contract deliverables are met.

KEY SELECTION CRITERIA
* Certificate (III or IV) or Diploma qualification in a relevant discipline (Ageing Support, Community Services, Health Promotion, etc.)
* Demonstrated 2+ years experience in a similar community‑based role
* High‑level written and verbal interpersonal and communication skills
* Ability to supervise and provide support and guidance to team members
* Experience in supporting behaviour change, motivational interviewing, and lifestyle modifications
* Ability to work autonomously and demonstrate strong reasoning and problem‑solving
* Strong understanding of primary care and chronic disease management
* High‑level organisational skills and computer literacy
* Well‑developed stakeholder engagement skills and approaches
* Unrestricted drivers license
* National NDIS Workers Screening check or current Yellow Card
* Experience working with older adult clients 65 years and older
* Experience with communities with low health literacy and socioeconomic disadvantage, and mental health and well‑being challenges
* Demonstrated experience in a multidisciplinary care environment
* Experience with the development, implementation, and evaluation of group programs
