Clinical care partner & care partners (rn, en and cert iv opportunities)

Right at Home ACT & Queanbeyan are growing and we are on the lookout for a new Clinical Care Partner and/or Care Partner. For the Clinical Care Partner role we are looking for a Registered Nurse with extensive Community Aged Care experience. For the Care Partner role we are seeking Enrolled Nurse or Cert IV qualification candidates.

Right at Home ACT & Queanbeyan provides a range of care services to assist people to live independently at home. We have a unique and internationally recognised system of delivering care that is very well received by our clients. Our office is in Mitchell ACT. We service the North side of ACT and Surrounding NSW areas.

Salary Range:

Registered nurse: $90,000-$130,000. Plus Super. Based on Experience.

Enrolled Nurse: $68,000-$90,000. Plus Super. Based on Experience.

Cert IV Qualification: $60,000-$70,000. Plus Super. Based on Experience.

About You

To be considered, you will need

* Experienced RN, Experienced EN or Experienced Cert IV (aged care experience essential)

* Current AHPRA registration and professional indemnity insurance (RN & EN).

* Sound knowledge regarding Aged Care Sector and NDIS (My Aged Care, Support at Home, Care Planning, Government Guidelines, Funding, Budgeting).

* Ability to deliver high quality care, working with complex care needs and a demonstrated ability to apply high level of clinical knowledge (including palliative care, dementia care, wound care, catheter care). (RN & EN)

* Current National Police Check (Mandatory)

* WWVP Card (Mandatory)

* Drivers Licence and Comprehensively Insured Private Vehicle (Mandatory)

* Current First Aid and CPR Certificates (Mandatory)

* NDIS Clearance

* Smart Phone

* Must have excellent time management and communication skills.

* Must be able to use several systems on computer, including client database, share point & outlook etc.

* Willingness to complete Right at Home Mandatory Training Modules.

* A willingness to help us grow our client base and referral network.

* Ability to learn on the job and pick up new systems quickly.

* Must be able to work as a team and support our care workers in the community.

* Ability to record detailed client budgets to ensure services are within funding.

Operating in a home care environment, you'll be responsible for ensuring that our clients are safe in their homes and support our clients to continue to remain as independent as possible. You'll liaise with potential clients, set up individualized care plans & budgets to meet their unique needs and build professional relationships in the health industry.

Reporting to our Managing Director, your role will be a combination of front line nursing work, client care management and providing assistance to our care team. This includes but will not be limited to, the following:

* Design tailored and meaningful client care plans that foster positive relationships between our clients and the Care Team.

* Meet with potential clients to consult on their options for quality home care services.

* Oversee client clinical care and maintain meticulous care documentation records.

* Oversee our Incident Management System and follow the process as per policy.

* Conduct nursing visits in the community.

* Assist in the management and development of a growing team of caregivers and nurses.

* Make it your mission to improve the quality of life for those we serve.

* Answer incoming calls and ensure timely follow up for clients.

* Prioritize diary and manage busy work load.

Please note that Clinical Tasks listed above are not in relation to Cert IV Care Partner role and are only in relation to RN and EN applicants. Cert IV Care Partner's will be required to work closely with the RN and will need sign off on particular documentation.
