Job Title: Community Care Professional
As a Community Care Professional, you will provide high-quality care to clients in the community. This role involves providing person-centred care, ensuring a safe environment, and applying clinical skills to improve quality of life.
This position requires:
* Assessment, planning, and implementation of client care within your scope of practice
* Consideration of physical, social, and psychological requirements
* Creation and updating of care plans to reflect developing needs
* Ensuring clinical quality compliance standards are met
* Collaboration with other multidisciplinary team members
You will spend most of your day on the road, completing scheduled community visits.

Required Skills and Qualifications:
* Full, unrestricted registration with the relevant professional body
* General nursing experience post-registration
* Excellent communication and clinical problem-solving skills
* Current driver's licence
* A roadworthy and insured vehicle for work purposes
* Confidence in managing time effectively, including autonomy
