Our organization offers a permanent part-time opportunity for a Care Manager to deliver person-centered case management services.
The successful candidate will be responsible for managing client cases, working within expected time frames, and liaising with internal stakeholders.

Required Skills and Qualifications
To be successful in this role, you will need:
* Demonstrated experience in the provision of Case Management services
* Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
* Driver's license
* Highly developed proficiency using computers including the Microsoft office suite with the ability to learn and adapt new databases
* Understanding of the financial principals of individualised client funding and experience in monitoring and reviewing individualised budgets
* An ability to work in a team and negotiate with internal and external stakeholders to ensure accountability for service delivery
* NDIS clearance (or willingness to obtain one)
* Police clearance (within the last 6 months Or willingness to obtain one)
