o AHP3 $108,347 - $116,216 p.a.

About the Role:

Join the Northern Adelaide Local Health Network (NALHN) as a Community Support Coordinator in our Older Persons Mental Health service.

The Older Persons Community Support Program (OPCS) Coordinator works across NALHN Older Persons Mental Health sites to support timely and effective discharge planning. The role is responsible for implementing the OPCS program and managing Wrap Around and other state based funding initiatives that support patient flow and improve outcomes for older people with mental health needs.

The coordinator works as part of a compassionate multidisciplinary team and provides community based support that assists older people to remain independent and well within their own homes and communities. The role includes comprehensive assessment, care coordination, advocacy and direct support to ensure consumers receive person centred care.

Experience working within a mental health setting is required, preferably within community mental health services.

Key responsibilities include:
• coordinating individualised support plans for older people with mental health conditions in the community
• completing comprehensive mental health and psychosocial assessments
• managing funding allocations
• liaising with families, carers, NGO providers and government services
• monitoring consumer progress and reviewing support plans
• providing clinical supervision and support to AHPs and RNs as required
• undertaking an education role with internal teams, community partners and external stakeholders
• facilitating access to appropriate community based services and resources

To be eligible for this position at the AHP3 classification, you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service and have experience of working within mental health setting, specifically:

* Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
* Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
* Appropriate Degree (Masters in Clinical or Health Psychology) or equivalent qualification as recognised by the PsyBA and general registration with PsyBA.

To be eligible for this position at the RN3 classification, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate. Must have completed a Mental Health course or holds a qualification in mental health practice.
