About The Role
The Clinical Nurse Consultant is a senior Registered Nurse who will provide clinical nursing leadership within the Preventative Integrated Care Service (PICS).
The position provides expert contemporary diabetes knowledge and skills using evidence‐based clinical practice to guide high‐quality, safe clinical services across West Moreton Health. It is also accountable for the clinical triage and referral coordination for the PICS model of care, working collaboratively with Nurse Practitioners and the Senior Medical Officer to ensure safe delivery of nursing care to chronic‐condition patients accepted to the PIC service.
About You
- Contemporary clinical knowledge in specialised sub‐acute and chronic disease management, working in a community setting with individuals, peers, and the healthcare team to achieve improved health outcomes across Preventative Health Services.
- Leadership in coordinating nursing and provision of health services across the continuum of care, delivering competent nursing care in complex situations and coaching patients to adopt self‐management strategies.
- Ability to adopt digital technology and utilise digital care solutions and telehealth as part of delivering and coordinating care closer to home.
- Advanced interpersonal, communication, and time‐management skills, demonstrating autonomous, sound, high‐level conceptual and critical‐thinking capabilities.
Mandatory Requirements
- Vaccinated against, and remain vaccinated against, certain vaccine‐preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16.
- Proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (AHPRA); certified copies must be provided to the supervisor/manager before commencement of clinical duties.
