Case Worker | Home Access for people with Disability | Red Hill
Employment Term: Full-time, maximum-term until 30 June 2028
Location: Red Hill
Remuneration: $44.58 p/h + super + NFP Salary Packaging (save tax and increase your take home pay)
About the role
As a Case Worker for the Home Access for people with Disability Program, you will work with clients to find and maintain suitable housing.
Responsibilities
* Assess client needs and develop housing plans.
* Coordinate with support services and landlords to facilitate tenancy and accessibility goals.
* Provide practical assistance and ongoing support to clients.
* Collaborate with stakeholders to help clients achieve greater independence and stability.
Qualifications
* Minimum Cert IV in Human Services or Disability, or Diploma in a related field.
* At least 2 years' experience in a relevant role.
* Current Working with Children Check (Blue Card).
* Current Driver's Licence.
* NDIS Worker Screening Check.
