Responsibilities
- Work in partnership with families to identify and achieve goals, focusing on the baby’s wellbeing.
- Conduct assessments in line with the Personal Health Record (PHR) schedule using evidence-based tools.
- Participate in weekly case reviews and consultations with a multi-disciplinary team.
- Provide guidance and support to build parental skills and confidence in a strengths-based model.
- Undertake sustained home visiting from pregnancy until the child’s second birthday, combining health and developmental assessments with ongoing family support.

About The Role
As a Child and Family Health Nurse/Midwife, you will work in close partnership with families to help them identify and achieve their goals while prioritising the baby’s wellbeing. The program involves sustained home visiting from pregnancy until the child’s second birthday, with health and developmental assessments and ongoing family support.
Requirements
- Current registration with AHPRA as a Registered Nurse and/or Midwife.
- Postgraduate qualifications and experience in Child and Family Health Nursing.
- Experience working with families using a partnership or strengths-based approach.
- Strong communication, organisational and teamwork skills.
- Ability to build safe, therapeutic relationships while maintaining professional boundaries.
- Valid NSW driver’s licence and willingness to travel for home visits.
