Main Responsibilities
We are seeking a skilled professional to carry out assessments and formulate support plans for service users, in accordance with national guidelines and prescribed timescales.
* Liaise with and take a professional lead in involving appropriate staff and agencies to meet the needs of service users.
* Organize packages of support tailored to individual service user needs.
The role requires a strong understanding of relevant legislation and Social Work experience.
In this role, you will work collaboratively with a team to provide high-quality services to service users.
