The goal of the organisation is to improve the efficiency and effectiveness of health services for people particularly those at risk of poor health outcomes, improve the coordination of health services, and increase access and quality support for people.

Country SA PHN is seeking a motivated and collaborative Primary Care Development Officer to support the delivery of initiatives that strengthen multidisciplinary care and enhance primary health services across regional and remote South Australia.
The Role
As the Primary Care Development Officer, you will:
* Support Project Delivery: assist in the coordination and implementation of activities focused on multidisciplinary team care, allied health, and workforce development.
* Engage Stakeholders: support collaboration with general practices, allied health providers, community service organisations, and advisory committees to enhance project outcomes.
* Contribute to Evaluation: assist in data collection and reporting processes to monitor project progress and impact.
* Develop Resources: help prepare practical tools and resources to support primary care providers in delivering integrated, patient-centred services.
* Coordinate Activities: organise meetings and support internal and external communications to ensure smooth delivery of primary care initiatives.
This position is part of the Health System Integration and Innovation team. It is expected that this position will remain flexible to meet the changing needs of the organisation and the community.
Key Responsibilities
* Assist in implementing projects aligned with primary care development and integration.
* Support engagement with key stakeholders and contribute to collaborative working relationships.
* Help identify service gaps and opportunities for improved patient and provider experiences.
* Maintain accurate documentation, reports, and project materials.
* Support the evaluation of activities through data collection and outcome tracking.
* Ensure culturally sensitive and inclusive communication and engagement across all tasks.
