What you'll be doing
As a Clinical Nurse Consultant in SESLHD you will play a pivotal role in clinical service and consultancy, clinical leadership, research, education and clinical services planning and management for Prince of Wales Hospital. You will contribute to patient flow initiatives and complex discharge planning within a multidisciplinary team.
Key Responsibilities :
* Lead and participate in multidisciplinary Long Length of Stay (LOS) meetings to review and manage patients with extended hospital stays, identifying and addressing barriers to timely discharge.
* Monitor and report on clinical performance metrics (KPIs) and implement improvement strategies.
* Develop and oversee complex discharge plans ensuring safe transitions from hospital to home or other care settings.
* Facilitate family meetings to discuss care plans, concerns and decision-making regarding discharge.
* Coordinate interfacility transfers to maintain patient safety and continuity of care.
This position is classified as Grade 2 under the NSW Public Health System Nurses' and Midwives' (State) Award, reflecting advanced practice and leadership responsibilities.
Selection Criteria
* Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
* Clinical Nurse Consultant/Clinical Midwife Consultant Grade 2 with at least 5 years post-registration experience (3 years in the specialty), preferably in an acute setting, with approved postgraduate nursing qualifications or equivalent experience as required by the employing public hospital or health service.
* Demonstrated ability to apply clinical expertise in a consultant role and work independently.
* Proven clinical leadership and ability to collaborate within a multidisciplinary framework with commitment to excellence in nursing practice.
* Evidence of critical thinking related to advanced evidence-based nursing care.
* Experience leading and implementing change, translating evidence-based research into practice within a person-centred framework.
* Strong interpersonal, written and verbal communication skills, and advanced computer skills for data analysis and reporting.
* Ability to contribute to strategic and clinical service planning with experience in reducing length of stay and navigating complex discharges and family dynamics.
