About the role: As Team Leader - Care Partner, you will play an integral role in the Support at Home program delivered by Suncare and ensure the achievement of positive customer and organisational outcomes through effective leadership of high performing Care Partner teams located in the Rockhampton region. More about the role: An established leader, you will empower your team through regular, effective communication and opportunities for professional growth and development. Play a pivotal role in growing your team through recruitment and onboarding of new Care Partners. Keeping your own skills current, from time to time, you will also maintain a small case load of Support at Home customers. Your team will deliver applicable funding programs within industry guidelines and standards, legislative requirements. You will drive best practice initiatives via your involvement in customer documentation audits, budget management, complaint management, and risk management activities. What you will bring to our team: Your highly developed customer service and interpersonal skills will be valued and essential in this role. You will demonstrate an ability to achieve efficient and effective customer outcomes and the ability to build positive, effective relationships with internal and external stakeholders, including working with a wide range of services and groups. You will also ideally possess: A tertiary qualification in social/human services or equivalent industry experience Demonstrated knowledge of community sector support services (including high level experience and knowledge of aged care funding and case management) Excellent communication and organisational skills Experience in leading, managing and supervising a coordination level team, with the ability to provide effective mentoring and coaching A valid Driver's Licence What we can offer: At Suncare our people really are our greatest asset.
